Affordable is a six-month question

The first-month price is the least informative number in GLP-1 telehealth. Almost every provider can make month one look affordable: a starting-dose price, a promotional discount, a waived consultation. What a patient can actually afford is determined by months three through six, when the dose has climbed, the promotion has ended, and the membership has been billed three to six times.

This page ranks providers by the lowest true cost in the first month — the honest version of "cheapest to start" — and then walks through what happens next, so a patient can budget for the plan rather than the promotion.

The first month, honestly

The true first-month cost is the starting-dose medication price plus the membership fee plus any consultation fee. On that measure, several providers cluster near $169: NexLife's month-to-month tirzepatide is $169 all-in; Mochi Health's reported $90 starting price plus $79 membership is also $169. Ivím Health's reported $133 plus $74.99 is about $208; Henry Meds' current program page lists no injectable compounded tirzepatide, so it is unranked for tirzepatide; Mochi's $90 is a first-month medication price ($129 with the introductory membership) that becomes $278 from month two. A patient who only ever took one month would find these providers roughly interchangeable on price.

Months two through six

From month two, the paths separate. A flat-rate provider bills the same number every month. A dose-based provider bills the next tier when the dose steps up — under the tirzepatide label, at weeks 5, 9, 13, 17 and 21 — so a patient who titrates on schedule is paying the highest tier by month five or six. Using the reported Mochi figures, the medication price rises from $90 toward $199 while the $79 membership continues, so the monthly bill moves from $169 toward $278. Six months on that path cost about $1,400; six months on NexLife's 6-month plan cost $894.

When paying more is the affordable choice

Cheaper is not always affordable, because a plan you abandon costs more than a plan you keep:

  • If you might stop after one or two months because of side effects, a month-to-month plan with no notice period beats a cheaper 12-month term. Weigh the no-commitment category result.
  • If you have insurance that covers Zepbound or Wegovy, the affordable option is the FDA-approved product through a program that handles prior authorization (Ro), where a copay can be lower than any cash price.
